If you did not receive Legend Rewards certificates with your recent Legend Brands purchase, please submit this form with your product purchase invoice, and the applicable Legend Rewards points will be deposited in your Legend Rewards account within 2 business days.

To ensure prompt deposit of points, you must enter the email, name and company name (if applicable) that matches your Legend Rewards account. If you are unable to log into your Legend Rewards account, please  email us.

To register truckmount purchases, complete the Truckmount Warranty Form prior to submitting this points request form.
